Freedom of Information Request Form

Under the Freedom of Infomation and Protection Privacy Act/Municipal Freedom of Information and Protection of Privacy Act.

Please Note: Once a Freedom of Information form is submitted, you will be expected to pay a $5 filing fee before your request is formally opened and the 30-day response time begins. Cash and debit payments must be paid in person at 56 Queen Street, Port Hope ON L1A 3Z9 or by cheque payable to the Municipality of Port Hope. Additional fees may apply to time required to redact and prepare documents for disclosure.

For personal information requests, you must provide proof of your identity before records containing your personal information are released to you.

If you are requesting records for another person, you must provide proof that you have the authority to act for that person (e.g. authorization form).

The personal information on this form is collected under legal authority of the Municipal Freedom of Information and Protection of Privacy Act, R.S.O 1990, Chapter M.56. The information will be used for the purpose of responding to your request.
