Upon completion of this form, staff will contact you to confirm the details of your request. All requests must be approved by the Supervisor, Cemeteries before payment will be accepted. Installations and plantings are completed annually in the Spring and in the Fall. Requests for Spring installation/plantings must be received by March 15 and requests for Fall installation/planting by August 1, as weather permits.
Please note the waterfront area is currently at capacity for donations. Limited select locations are available along the Riverwalk and in a variety of municipal parks.
Personal information is collected on this form under the authority of the Municipal Act and the Municipal Freedom of Information and Protection Act (MFIPPA) and will be used only to facilitate the municipal commemorative tree, bench and picnic table program. Residents’ personal information will only be kept for as long as necessary to fulfill the purposes for which it is collected.
