The Municipality requires the following docuemnts for all events prior to final approval:

  1. Site Plan: please provide a detailed site plan illustrating all physical objects of your proposed event set up.
  2. Emergency Plan: please provide an event specific emergency plan.
  3. Insurance: please provide appropriate liability insurnace coverage with the Corporation of the Municipality of Port Hope named as an additional insured.
  4. Other supporting docuements: provide any additional Municipal forms, sample timelines etc.

I acknowledge that the information on this event application form is complete and accurate to the best of my ability. I commit to the responsibility of advising the Municipality of Port Hope of any changes to the nature or details of my event at any time after this form is submitted.
