Describe your organization by selecting the descriptor that best defines your organizational structure. <br><br>A Non-Profit Applicant is an individual or an organization who is an incorporated non-profit or is raising funds to support charitable initiatives. <br><br>A Commercial Applicant is an individual or an organization with revenue-generating goals for their event on Municipal property. <br><br>Event applicants may be asked to supply proof of organizational status or affiliation with a non-profit organization.
Annual Event?
Please indicate if your event is an annual event.
Please indicate the anniversary your event is celebrating this year. Example: 4th annual.
Indicate the date your event will take place, or in the case of multi-day event, the first day of your event.
Indicate the time your event will begin (be open to event attendees).
Indicate the date your event will conclude.
Indicate the time of day your event will end.
Indicate the date in which you hope to begin event setup at your requested event site.
Indicate the time you request to begin setup at your requested event site.
Indicate the date your event cleanup, or tear down, will be finished.
Indicate the time of day your event cleanup, or tear down, will be finished.
Indicate the venues, locations, facilities, that you wish to host your event.
Indicate the number of events attendees you expect to attend.
Indicate the price of admission to attend your event.
Provide an overview of your event proposal.
Indicate your target audience. Include demographics and an overview of your expected audience.
Select all elements included in your event overview and proposal.<br><br>Event components may require the submission of additional forms, fees, maps and/or proof of compliance. Please refer to the Special Event Planning Guide to review requirements for the following elements.

The Municipality requires the following documents for all events prior to final approval:

  1. Site Plan: please provide a detailed site plan illustrating all physical objects of your proposed event set up.
  2. Safety Plan: please provide an event specific emergency and safety plan.
  3. Insurance: please provide appropriate liability insurnace coverage with the Corporation of the Municipality of Port Hope named as an additional insured.
  4. Other supporting docuements: provide any additional Municipal forms, sample timelines, draft promotion etc.
Please provide a site plan for your proposed event to help clearly communicate your event proposal and location request. A draft is acceptable at the time of application submission, as the site plan will be developed throughout the event review process,

An event specific emergency and safety plan does not have to be finalized at time of event application submission. Event applicants are encouraged to submit a draft. Prior to your event taking place, a final version will be required to be reviewed and approved by the Municipality.

Event Insurance does not have to be obtained at time of event application submission. <br><br>Evidence of appropriate coverage must be provided prior to the approved event. All Special Events require appropriate liability insurance coverage in the amount of $5,000,000 with the Corporation of the Municipality of Port Hope named as an additional insured.
