Wall of Honour Nominee Form

The Athletic Wall of Honour provides a means for the community to honour an athlete or athletic supporter by recognizing significant achievements in sport. Make a nomination by completing the form below. All nominations will be reviewed by staff and the PRC Advisory Committee, who have set the criterion of the program. Nominations that do not meet the criteria may be eligable for recognition in the Sports Kiosk. Incomplete nominations will be returned to the nominator.

Criteria for Nomination of an Athlete:

An Athlete shall be defined as someone who has demonstrated character, sportsmanship, and integrity, as well as outstanding ability in their athletic event, and has brought honour to Port Hope. Candidates for Athlete Nominations must:

• have been born in Port Hope, or lived in Port Hope for a minimum of two (2) years OR participated in sport in Port Hope for two (2) consecutive seasons;
• participated in or recognized for Provincial, National, or International achievements;
• have been retired from active participation in sport for a minimum of two (2) years, dating from the last season of participation in sport. In considering nominations for deceased candidates, those responsible have the option of waiving the two-year waiting period.

Criteria for Nomination of a Supporter:

Supporters shall be defined as officials, administrators, sports medicine personnel, media personnel, coaches, and others who work for sport. Candidates for Supporter Nominations must:

• have been born in Port Hope, or lived in Port Hope for a minimum of two (2) years OR participated in sport in Port Hope for two (2) consecutive seasons;
• recognized for National or International achievements;
• have been retired from active participation in sport for a minimum of two (2) years, dating from their last participation in their area of experitise. In considering nominations for deceased candidates, those responsible have the option of waiving the two-year waiting period.

The Municipality of Port Hope reserves the right to manage the content at its discretion and by submitting this content I agree to allow the Municipality of Port Hope permission to use it as part of the Wall of Honour program and its promotions.
